Parkside Family Medicine is excited to welcome you as a new patient. We are committed to providing you and your family with the highest quality care. We want to be your teammate in meeting your healthcare goals and needs while providing preventative care and disease management along the way.

We make scheduling your first appointment easy. You can start the process one of two ways. Either use the button below to fill out the new patient paperwork so that one of our team members can call you, or you can call our office at 864.516.1783 and ask to become a new patient.

What to expect

Your First Visit

Your first appointment at Parkside Family Medicine will be a consultation between you and your new primary care provider. We will spend time getting to know you, your medical history, and your personal healthcare goals. Then, as a team, our providers use the information and insights from your first appointment to build a personalized healthcare plan for you.

Please arrive 15 minutes early to your first visit with us to allow time to check in and complete any necessary paperwork before your appointment. Our New Patient Registration is available in our forms section for you to complete at your convenience before your appointment. Please bring your driver’s license and insurance card to your initial visit.

General Patient Information

Patient Experience

We desire to be a medical home for you. You will be supported by a dedicated team of health professionals, working alongside you to achieve your health goals using the best, evidence based medicine and resources available today. We want to empower you to take responsibility for your health and give you the self-management support that you need to succeed.

After Hours Emergencies

For all medical emergencies, please call 911. Our regular office hours are 7:45am – 5:00pm, Monday through Friday. After hours, one of our providers is on call. Please call our office to speak with our nurse triage line.

Insurance Overview

We accept most private insurance plans. Please be aware that your co-pay or deductible is due at the time of service. It is your responsibility to inform us of changes to your insurance plan. Although we file claims for you, you are still responsible for your bill, regardless of the amount your insurance pays, except in cases of pre-negotiated insurance agreements and where legally prohibited.

If your insurance requires a referral for a specialist or prior approval for testing, our team will make every effort to provide your insurance with the necessary information.

If you do not have insurance, payment is expected at the time you receive services. A discount is given for self-pay patients.

Payment Policy

Proof of Insurance:
You must provide a copy of your current health insurance card as proof of insurance. If you fail to provide us with the correct insurance information at the time of service, you may be responsible for the balance of your claim. If your insurance changes between visits, please notify us before your next visit so we can make the appropriate changes to ensure you receive your maximum benefits.

Copayments / Outstanding Balances:
If applicable, all co-pays and outstanding account balances must be paid at the time of each visit. Claim Submission / Billing Statements:We will submit your claims to your insurance provider following each visit to our practice. After processing with your insurance provider, we will send you a billing statement from Tribe513 for any remaining uncovered charges. We ask that you provide payment in full for all services rendered immediately upon receipt of each billing statement.

Monthly Billing Statement:
After your insurance company pays Parkside Family Medicine you will receive a monthly billing statement, which indicates your balance due and/or deductible due. These amounts are payable to Parkside Family Medicine. The balance due is payable in full within 10 days of receipt of the monthly billing statement. If you have questions about your account, please call 864.516.1783.

Non-Covered Services:
Please be aware that some – and perhaps all – of the services you receive may not be covered by your insurance policy. In this event, you agree to be financially responsible for all uncovered charges. Please contact your insurance company with questions or concerns regarding your coverage.

Self-Pay:
While we participate in most insurance plans, if we do not accept your insurance plan or you do not have insurance, payment in full is expected at each visit, subject to a self-pay discount.

Partial Payments:
Partial payments will not be accepted unless otherwise negotiated with our billing department.

Non-Payment:
Please be aware that if your account goes unpaid beyond 30 days of the date of your initial billing statement, we may refer your account to a collection agency, and you will be discharged as a patient from all Tribe513 practices and related entities. Should this occur, you will be responsible for any collection or legal cost associated with collecting on your account. We understand that everyone’s situation is different and financial hardships may occur. Please contact our billing department should you find yourself in a hardship position and we will do our best to work with you to the extent we are able.

Missed Appointments

To achieve the best appointment availability for our patients, we have a policy for missed appointments. There will be a $50 charge added to the account for a missed appointment. Three missed appointments within a 12-month period will result in eligibility for discharge from the practice for the family. We understand the potential for unforeseen circumstances that can arise that may cause a late arrival or missed appointment. If this happens, please call us as soon as possible so we can change your appointment status accordingly and make it available for another patient.

Cancellations

Our policy is to charge $25 for previously scheduled appointments that are canceled less than 24 hours prior to their scheduled time/date. These charges will be your responsibility and billed directly to you, and not your insurance company. Please help us serve you better by keeping your regularly scheduled appointments.

Forms of Payment

Parkside Family Medicine accepts payment by cash, check, money orders, Visa, MasterCard, American Express, Discover, and debit cards bearing these logos. Payment is expected at the time of service.

Medication Refill

Please remember to bring in your current prescription bottles to every visit. Established patients can update their medication list in our Patient Portal prior to their visit and bring a copy in with them.

Holidays

Parkside Family Medicine is closed so our team can enjoy the following holidays with their families. However, we continue our 24-hour availability during holidays for urgent medical needs through our answering service.

Patient Portal FAQs

Is having online access secure?

Yes, it is protected with a username and password/PIN number of your choice and through SSL encryption, making it as secure as online banking.

How do I sign up?

You should receive a username and password from our practice via email that you can print out and customize after your first appointment. Access can be made on any web browser or through the Healow smartphone app.

Do I have to pay to get access to my patient portal?

Access to your desktop Portal and the Healow™ mobile app are free. We share this resource with our patients to give them better access to their own healthcare information.

What can I access through my patient portal?

You’re able to message a nurse, view lab results, view medications, pay your bill, and more.

What if I forget my username and password?

The app and desktop portal allow you to reset your username and password. Just select the “trouble logging in” prompt and follow the steps provided.

What if I need technical support?

Technical support is built into the web portal and the app, or you can ask a Parkside Family Medicine team member for assistance.
